My Dietitian can computer analyse your diet for a typical day to ensure that you are having:

• a healthy proportion of protein, fat and carbohydrate,

• an option for long-term nutitional support .

	Filling in the forms may take you approximately 25 minutes. Please complete the forms as accurately as you can. The information you will be asked will include your food intake for a typical 24-hour period. As you fill out the forms, there will be sample forms available to assist you.

	After we receive your completed forms, we will send you a computerised dietary analysis with recommendations similar to this sample (PDF, 199 kB  ) via email and/or the post.
	We will phone you or send you an e-mail should we need to clarify any of the information you provide.
