The My Dietitian Weight Control Program is personal, practical, professional and affordable - For Optimal Health and Lifestyle.

The weight control program offered by My Dietitian will provide you with:

• Computer analysis of your usual food intake during a typical day
• A recommended meal plan, outlining:
  • Your energy, vitamin and mineral requirements
  • The right types of fats to protect your heart
  • Improved antioxidant intake
• Long-term follow-up nutritional support (optional)
• One-week personal meal plans, including recipes and serve sizes (optional)

The Weight Control Program is designed by an experienced and professional dietitian to suit your individual requirements and weight goals. Long-term nutritional support is also available for as long as you need or request it.

	Filling in the forms may take you approximately 25 minutes. Please complete the forms as accurately as you can. The information you will be asked will include your food intake for a typical 24-hour period. As you fill out the forms, there will be sample forms available to assist you.

	After we receive your completed forms, we will send you a computerised dietary analysis with recommendations similar to this sample (PDF, 135 kB  ) via email and/or the post.
